Mission

STRATIFYHF aims to develop, validate and implement the first AI-based, decision support system (DSS) for risk stratification, early diagnosis, and disease progression assessment in HF to accommodate primary and secondary care clinical needs. The DSS will integrate patient-specific demographic and clinical data using existing and novel technologies and establish AI-based tools for risk stratification and HF prediction using machine learning.

Expected Impact

The collaboration between STRATIFYHF and SmartCHANGE will amplify efforts to predict and manage risks associated with chronic diseases. By leveraging STRATIFYHF’s expertise in heart failure and SmartCHANGE’s focus on youth-centric health promotion, the partnership will promote the integration of predictive tools across different demographics. This collaboration will also enhance stakeholder engagement, fostering cross-project learning on ethical AI implementation and long-term health interventions. The synergy will facilitate networking among researchers, healthcare professionals, policymakers, and industry stakeholders from both projects, laying the groundwork for future collaborations, joint research initiatives, and cross-disciplinary partnerships.
